LUDOC-11 misc: replace non-ASCII control chars

LUDOC-11 misc: replace non-ASCII control chars

Replace a large number of non-ASCII characters.  This includes
the use of ' or " where needed instead of "fancy quotes",
replacing m-dash with regular hyphens, using <literal> tags instead
of quotes, etc.

Reformat affected lines to wrap at 80 columns, using "semantic"
line breaks after comma or period where appropriate.

In a few cases, confusing or incorrect text was replaced.
