LUDOC-155 lfsck: LFSCK Phase II Additions

Added information about MDT-OST file layout consistency
checking, LFSCK phase II, to the manual section about
LFSCK.

Also cleanup up a few typos, table column issues and
minor corrections (including LUDOC-182).
